Life-Threatening Student Medical Alerts
Parents/Guardians are responsible for advising the school office when a student has a life-threatening medical condition.
School District #23 Medical Alert Planning Form (F436.2) must be completed and returned to the school office. The Medical Alert Planning form must be reviewed annually in September by a parent/guardian.
If medication is required, School District #23 Request for Administration at School Form (F436.1) must be completed and returned to the school office. Parent/Guardian must update in September.
A copy of the prescription may be provided in lieu of a physician's signature.
Epi-Pens and/or Medications kept in the school Medical Room must be picked up at the end of the school year.
